Escalations are not agent problems. They are unlabeled friction.
A lead who reviews escalations by "who dropped the ball" will coach the same people every month and change nothing in the queue.
The pattern is almost always one of a short list: a policy gap, a handoff drop, missing authority, a repeat contact that should have been closed, a slow response the customer already paid for.
Name the friction for 30 days. Do not edit the categories in week one. In week two, anything that shows up four times is a system. In week three, coach from the entries, not a feeling. In week four, write what actually changed in the queue.
That is the whole method. The map is a log, not a dashboard.
