Why your clinic loses patients to the guy with 320 Google reviews

Ran the numbers across a few Edmonton medical clinics this year and the pattern is brutal: it's almost never the better clinic that wins the Map Pack. It's the one with more recent 5-star reviews.


Here's the mechanism most front desks miss — Google's local ranking algorithm weighs three things hard: proximity, relevance, and prominence (review count + review recency + review velocity). A clinic with 25 reviews from 2 years ago loses to a newer competitor pushing 15-20 fresh reviews a month, even with a lower total rating.


The reason most practices stall out at 25-40 reviews forever: asking a patient for a review face-to-face at checkout is awkward, and front-desk staff — understandably — deprioritize it under real clinical workload. It's not a training problem, it's a friction problem.


What actually moves the needle:

  • Trigger the ask 1-2 hours after the visit, not at checkout. Patients are calmer, off the clock, and more likely to complete a 1-tap flow.

  • Respond publicly to every review within a few hours, and work a local keyword into the reply (procedure + neighborhood). Google indexes your own responses too.

  • Keep your GBP category and NAP (name/address/phone) dead consistent across every directory — inconsistency quietly tanks relevance signals.
