5 Things I Wish I Knew Before Starting My IMG Journey

If you're an International Medical Graduate (IMG) figuring out how to practice medicine abroad, this post is for you. These are hard-won lessons from real IMG experiences.


1. Your Country of Practice Should Match Your Life Goals — Not Just Prestige


Everyone defaults to "I want to go to the US" without asking deeper questions:

  • Do you want to be near family? → UK or Europe might cut your timeline in half

  • Do you want earning potential above all? → US residency pays off long-term but costs 3-5 years of intense prep

  • Do you want work-life balance? → Countries like Germany, Australia, and Scandinavia score higher

  • Do you need to start earning quickly? → UK's NHS route can get you working within 12-18 months


There is no "best" country. There is only the best country for YOU.

3. Immigration Is Half the Battle


Nobody talks about this enough. Passing the exam is ONE hurdle. Getting a visa, work permit, and eventually permanent residency is a whole separate journey.


Key examples:

  • USA: J1 visa → 3-year waiver requirement → H1B or Green Card. Budget 7-10 years total

  • UK: Skilled Worker Visa (formerly Tier 2) → ILR after 5 years → Citizenship

  • Germany: Job seeker visa → Approbation → Blue Card → Settlement permit after 21 months (with B1 German)


Start your immigration research before you pick your exam. Some paths close if you make the wrong sequence of decisions.


4. Start Clinical Experience Early


Whether it's observerships, externships, or clinical attachments — start early. In the US especially, having US Clinical Experience (USCE) on your residency application is nearly non-negotiable for IMGs.


For the UK, getting NHS experience through clinical attachments or trust-grade positions makes you a stronger candidate for specialty training.


5. Community Is Everything


The IMG journey is lonely. Most of your med school classmates won't understand your path. Find a community of people going through the same thing — it makes the difference between burning out and pushing through.
