The USMLE stem already told you. Most people read past it.
Long stems are not there to exhaust you.
They are there to hide one specific clue in a pile of expected details.
Look at the graphic. Fever, weight loss, and a cough could be a dozen things. Recent immigrant, night sweats, and painless cervical lymphadenopathy are not flavor text. That is the test telling you where to look.
How to read a vignette in 20 seconds
Scan for the unusual detail first. Age, travel, occupation, a timed lab, a physical that does not fit.
Then read the actual question. "Next step" and "most likely" are different exams.
Cross out answers that treat the generic story instead of the specific clue.
If a detail did not need to be in the stem, it is probably the point of the stem.
