The 3 NEMT Contracts That Pay the Most (And How to Get Them)

Most people getting into non-emergency medical transportation chase Medicaid contracts first. Makes sense — it's the biggest payer in the space.

But here's what nobody tells you: Medicaid is the lowest-paying contract type in NEMT.

The three highest-paying contract categories:

1. Private insurance / managed care organizations (MCOs)

These pay 2-3x what Medicaid pays per trip. MCOs are actively looking for reliable providers because their current ones no-show constantly. If you can show up on time with a clean vehicle, you're already ahead of 80% of the competition.

2. Hospital & facility direct contracts

Dialysis centers, rehab facilities, and hospitals need consistent transport for recurring patients. These are gold — predictable routes, predictable income. One dialysis center contract can be worth $8,000-15,000/month.

3. Veterans Affairs (VA) transportation

The VA contracts out non-emergency transport for veterans. The rates are strong, the volume is steady, and most providers don't even know these contracts exist.

The play: Start with one MCO contract and one facility contract. Build your reliability track record. Then stack from there.

I break down exactly how to land each of these inside my free course. No fluff, just the actual steps I used to build my own NEMT operation.

The business nobody talks about that's printing money in every city

There's a business that exists in every single city in America that most entrepreneurs have never even heard of.

Non-Emergency Medical Transportation (NEMT).

Every day, thousands of people need rides to dialysis, rehab, doctor's appointments, and therapy sessions. Medicaid and insurance companies pay for these rides. The demand is insane and it's only growing as the population ages.

Here's what most people don't realize:

  • You can start with one vehicle you already own

  • Contracts come from Medicaid brokers, rehab facilities, and hospitals — not random customers

  • The average NEMT vehicle generates $4,000-$8,000/month in revenue

  • Once you have contracts, the income is recurring and predictable

I've been doing client transportation for rehab facilities and I built this community to show people exactly how to get started — the licensing, the insurance, how to land your first contract, and how to scale from one van to a full fleet.

If you've been looking for a real business that doesn't require a huge upfront investment or a social media following, this is it.
