• Two meds that are fine alone and a problem together

• "Take with food" when nobody said which food, or that coffee counts

• A new dose that looks like the old one

• Side effects people white-knuckle for weeks because they think that's just how it is

A real medication review is slower. Drug list, timing, what else you take (including supplements), and what you were told vs what the bottle says.
