If your agent can do any of these, put a human in front of it:
Move or refund money
Delete or overwrite data
Change permissions or access
Publish something public
Send a message to a customer for the first time
What other actions would you add to this list? Drop them in the comments.

Before your AI agent goes live, check these five things:
Every action the agent can take is listed and classified (low, medium, high risk)
Every high-risk action has a named approver
The agent cannot bypass the approval step (least permissions)
Every decision is logged: who, when, what
A kill switch exists and you have tested it

Most teams shipping AI agents never decide which actions need human approval. That is how an agent ends up sending the wrong email, issuing a refund or deleting data with nobody looking.
A simple way to fix it, in three tiers:
Low risk: reversible, internal (read, summarize, draft). Let the agent run, and log it.
Medium risk: visible outside the company (emails to customers, CRM edits). A human approves before it goes out.
High risk: irreversible, financial or legal (payments, deletions, permission changes). Always a named approver, a deadline and a logged decision.
Rule of thumb: if an action is not classified yet, treat it as high risk.
