Overnight high-alert checks break in the gaps, not in the policy

Hospital policy already says what a high-alert check requires. Overnight is where that policy meets interruptions, thin backup, and a clock that does not pause.

The failure is rarely “I did not know the five rights.” It is the round that gets cut in half by a call light, a changed order, or a handoff with three pending checks still open. That is when informal workarounds show up: finishing from memory, skipping the second look because the patient “always gets this,” or documenting as if the sequence was clean.

A more reliable overnight approach is a short pause you can restart:

  1. Stop before the first check, even if you have given this med a hundred times.

  2. Verify the medication in front of you, not the one you expected.

  3. If you were interrupted, restart dose verification from the beginning. Do not resume mid-step.

  4. Look at the patient context before the time pressure decides for you.

Use a simple traffic light: green is a routine check, yellow is a clarification pause, red is immediate escalation. Carry pending checks across the handoff in writing. Raise a concern as a process issue, not as personal failure.

None of this replaces your facility’s policies. It is a way to keep identity, dose, timing, and patient context visible when the night does not go as planned.

The guide and the verification kit are live if you want the full sequence, the restart SOP, and the checklists in one place.
