You don't have a vocabulary problem. You have an output problem.

Apps train recognition. A green checkmark for matching "manzana" to a picture of an apple. That's input. Fluency is output — retrieving the word under pressure, with a face looking at you, while your brain is also trying not to look stupid.
Those are different skills. Recognition does not become production just because you did it 400 days in a row.
What actually moves the needle:
Speak before you feel ready. The first 20 hours of ugly output do more than the next 200 hours of passive listening.
Repeat the same 50 sentences until they're automatic. Novelty feels like progress. Automaticity is progress.
Get corrected in real time. An app never interrupts you. A person does. That's the whole point.
If your current "practice" never makes you sweat, it isn't practice.
