Most creators lose 60-80% of potential signups because of avoidable form mistakes. Here's what I see constantly:

1. Too many fields. Every field you add drops conversions by ~5%. Name + email is enough for most use cases. Ask for more later.

2. No mobile optimization. Over half your visitors are on phones. If your form doesn't work flawlessly on mobile, you're leaving money on the table.

3. Generic "Submit" button. "Get my free guide" converts 30% better than "Submit." Be specific about what happens next.

4. No progress indicator on multi-step forms. People abandon what they can't see the end of. Show them they're 2/3 done.

5. Asking for phone numbers without context. Nobody wants to give you their number unless you explain WHY. Add a one-liner: "We'll text you the download link."
