3. No automated follow-up sequence. Most leads don't convert on contact #1 — they convert on contact #3 or #4, and almost nobody follows up that many times manually.

4. No visibility into what's falling through the cracks. If you don't have a report of "leads with no follow-up in 24 hours," you don't actually know how many you're losing.
