Most scribes lose time in the Assessment, not the HPI.

Keep S to one line: the complaint plus duration.

Keep O to vitals, meds, and the two exam findings the doctor actually said.

Write A as the working diagnosis the doctor named — not the one you inferred.

Write P as orders, follow-up, and the exact words for return precautions.

If you did not hear a diagnosis, leave A blank and flag it. Guessing is how notes get rewritten at 6pm.

Dictate the O while the doctor is still in the room. Type A and P after they leave. That split is faster than trying to catch the whole note live.
